Your First Visit

Welcome to Gloria Gates CARE Centers! Here’s a quick overview of what to expect during your first primary care visit.

What to Expect During Your Comprehensive Primary Care Visit

At Gloria Gates CARE Centers, we’re committed to providing comprehensive, patient-centered care that addresses all aspects of your health and well-being. For a list of services that WOWCare Health does NOT cover, please refer to the Subscription Guide.

Here’s what you can expect during your primary care visit:

Check-in and Initial Screening

When you arrive, you’ll be greeted by our front desk staff. They will verify your information and may ask you to complete some brief paperwork. This could include questions about your health and any changes since your last visit. If you have an email on file, these may be sent to you to complete electronically at home before your visit.

Meeting with a Medical Assistant

A medical assistant will then escort you to an exam room and review your medical history:

  • Take your vital signs (blood pressure, temperature, etc.)
  • Review your current medications
  • Ask about any health concerns you’d like to discuss with your provider
  • Conduct any necessary screenings based on your age, gender, and health history

Visit with Your Primary Care Provider

Your provider will review the information collected by the medical assistant and discuss your health concerns with you.

They may:

  • Perform a physical exam
  • Order or review lab tests
  • Adjust medications if needed
  • Provide education on managing your health conditions
  • Discuss preventive care recommendations

Your provider will take the time to listen to your concerns and work with you to develop a personalized care plan.

Care Advocate Consultation

After seeing your provider, you may meet with a CARE Advocate. This team member is dedicated to helping you navigate the healthcare system and address any social or economic factors impacting your health. They can:

  • Help coordinate referrals to specialists or community services
  • Assist with obtaining needed medications or medical equipment
  • Connect you with resources for food, housing, transportation, or other needs
  • Provide education on managing chronic conditions
  • Help you set and achieve health-related goals

Additional On-Site Services

Depending on your needs, you may also have access to other services during your visit, such as:

  • Behavioral health consultation: Meet with a mental health professional for counseling or support
  • Clinical pharmacist: Review your medications and discuss any concerns or questions
  • Dietitian: Get personalized advice on healthy eating
  • Physical therapy: Receive an assessment or treatment for musculoskeletal issues

Follow-Up and Care Coordination

Before you leave, our team will:

  • Schedule any needed follow-up appointments
  • Provide you with a summary of your visit and care plan, including any orders
  • Ensure you understand any new medications or treatment instructions

Our primary care model is designed to provide you with comprehensive, coordinated care that addresses all aspects of your health. We strive to make each visit efficient, thorough, and responsive to your individual needs.

After Your Visit

Our care doesn’t end when you leave the clinic. You can expect:

  • Follow-up calls or messages to check on your progress
  • Calls from your care team with test results
  • Ongoing support from your Care Advocate as needed

At Gloria Gates CARE Centers, we’re committed to being your partner in health. Our team-based approach ensures that you receive the care and support you need to achieve your best possible health outcomes.

***WOWCare Health is NOT health insurance or workers’ compensation. WOWCare Health membership only covers the limited primary care services and medications that we provide to you and does not cover all healthcare needs, such as surgery and hospitalization.
